Dr. Silber is an out-of-network provider and does not participate in any insurance plans. Fees will be discussed prior to your first appointment.
About insurance
Although Dr. Silber does not participate with any insurance plans, upon receipt of your direct payment, you will be provided with a paid invoice (superbill) that you may submit to your insurance carrier for reimbursement.
Insurance tip sheet
You may contact your insurance company for details regarding your out-of-network benefits before starting treatment (including deductibles, out-of-pocket maximum, percent reimbursement), and regarding coverage of prescriptions and lab tests, often covered, but with varying out-of-pocket costs depending on the pharmacies and labs you use.
Your insurance company may ask you for billing codes to provide you more information about reimbursement rates.
